Data controller. Dr. M. Enrique Guzmán García, with offices at Hospital Sierra Alta, Suite 315, Sierra de la Campana 3701, Z.C. 31207, Chihuahua, Mexico, is responsible for the processing of your personal data under the Mexican Federal Law on the Protection of Personal Data Held by Private Parties (LFPDPPP).

Data collected. Identification and contact details; billing data; and sensitive health data: clinical history, medical background, physical examination, studies, clinical photography and operative notes. Processing sensitive data requires your express written consent.

Primary purposes. Creating and keeping your clinical record under Mexican standard NOM-004-SSA3-2012; assessment, planning and performance of treatment; postoperative follow-up; billing; and appointment communication.

Clinical photography. Photographs form part of your record and are needed to plan and compare the result. Publishing them for promotional purposes requires an additional, specific written consent, which you may refuse without affecting your care and revoke at any time.

Transfers. Only to the hospital, laboratory or pathology service involved in your care, and to your insurer if you request it. Data is never transferred for third party marketing.

Your rights. You may Access, Rectify, Cancel or Object to the processing of your data and revoke your consent, by submitting a request with photo identification at the office.

About the images on this website. The facility photographs and the gallery comparisons are computer-generated demonstration images; they do not correspond to patients or to real results from this practice.
